Family Support Plan

Supporting a loved one with diverse abilities is deeply meaningful—but it can also feel overwhelming when systems, services, and daily life demands intersect.

Supporting Families as Caregivers

The Focus on Freedom Family Support Plan supports families as caregivers by creating a clear, goal-driven plan that builds skills, confidence, and sustainable routines at home and in the community.

Through collaboration and structured planning, we help families identify priorities, build strategies, and strengthen the systems that support their loved one’s growth and independence while enhancing the quality of life as a family.

This service is available through Children’s Long-Term Support (CLTS) funding and is designed specifically to support unpaid caregivers in building the knowledge, tools, and structure needed to support their loved one effectively.

What Is the Family Support Plan?

The Family Support Plan is a structured caregiver support service focused on helping families build practical systems that support everyday life.

Rather than offering one-time advice or overwhelming families with large amounts of information at once, this program focuses on building support one nugget at a time.

Strategies are broken down into small, manageable steps so families can learn, apply, and build sustainable systems that work within their home, school, and community environments.

Each plan focuses on:

Strengthening communication and understanding
Creating sustainable routines and systems
Building independence skills for the individual
Helping caregivers feel confident navigating supports and services
Aligning home, school, and community expectations

Every family’s situation is different, which is why each plan is differentiated to reflect the family’s goals, needs, and current stage of support.

How the Program Works

Family Support Plans are completed within a six-month service window and are designed to remain flexible based on a family’s goals, availability, and priorities.

Families May Choose From Three Service Levels:

10 Hours within 6 Months

15 Hours within 6 Months

20 Hours within 6 Months

Together, we determine the level that best supports your family at this point in your journey.

A Goal-Driven Process

Each Family Support Plan includes:

At Focus on Freedom, we begin with awareness. Before strategies are introduced, we take time to understand what is happening within routines, communication, regulation, and daily life. This awareness helps families notice patterns, strengths, and areas that may be creating stress or overwhelm.

From there, we build practical strategies one nugget at a time. By introducing small, manageable steps, families are able to apply and strengthen new supports gradually.

This process allows families to build sustainable systems over time, rather than feeling inundated with information or temporary solutions.

Designed for Families as Caregivers

At Focus on Freedom, we recognize that caregivers are often coordinating multiple systems while also supporting daily life at home.

This service acknowledges the essential role families play and provides the structure, tools, and support needed to strengthen both the individual with diverse abilities and the family system as a whole.

When caregivers feel supported and equipped with the right tools, everyone benefits.
