Supporting Families as Caregivers
The Focus on Freedom Family Support Plan supports families as caregivers by creating a clear, goal-driven plan that builds skills, confidence, and sustainable routines at home and in the community.
Through collaboration and structured planning, we help families identify priorities, build strategies, and strengthen the systems that support their loved one’s growth and independence while enhancing the quality of life as a family.
This service is available through Children’s Long-Term Support (CLTS) funding and is designed specifically to support unpaid caregivers in building the knowledge, tools, and structure needed to support their loved one effectively.
What Is the Family Support Plan?
The Family Support Plan is a structured caregiver support service focused on helping families build practical systems that support everyday life.
Rather than offering one-time advice or overwhelming families with large amounts of information at once, this program focuses on building support one nugget at a time.
Strategies are broken down into small, manageable steps so families can learn, apply, and build sustainable systems that work within their home, school, and community environments.

Every family’s situation is different, which is why each plan is differentiated to reflect the family’s goals, needs, and current stage of support.
How the Program Works
Family Support Plans are completed within a six-month service window and are designed to remain flexible based on a family’s goals, availability, and priorities.
Families May Choose From Three Service Levels:
10 Hours within 6 Months
- Focused support around a specific area of need
- Strategy development and implementation planning
- Ideal for families seeking targeted guidance
15 Hours within 6 Months
- Expanded support across multiple areas of family routines and systems
- Continued collaboration to refine strategies
- Increased opportunity to apply tools and supports
20 Hours within 6 Months
- Comprehensive support across several areas of development
- Continued collaboration and strategy development
- Deeper implementation of family support systems

Areas of Support
While each Family Support Plan is differentiated, some of the common areas of focus may include:
Communication & Understanding
- Understanding how your loved one communicates
- Supporting both verbal and nonverbal communication
- Using visuals, AAC, and other communication supports
Daily Living Skills
- Creating routines that support independence
- Supporting hygiene, organization, and household skills
- Breaking daily tasks into manageable steps
Emotional Regulation
- Recognizing triggers and sensory needs
- Creating predictable systems that reduce overwhelm
- Building strategies that support regulation at home and in the community
Executive Function & Organization
- Supporting transitions and task initiation
- Creating visual systems and routines
- Reducing decision fatigue for caregivers and individuals
School & Service Collaboration
- Understanding systems such as evaluations, IEPs, and services
- Preparing for meetings and conversations with teams
- Strengthening collaboration between home, school, and providers

- Reflection and adjustments as the plan evolves
At Focus on Freedom, we begin with awareness. Before strategies are introduced, we take time to understand what is happening within routines, communication, regulation, and daily life. This awareness helps families notice patterns, strengths, and areas that may be creating stress or overwhelm.
From there, we build practical strategies one nugget at a time. By introducing small, manageable steps, families are able to apply and strengthen new supports gradually.
This process allows families to build sustainable systems over time, rather than feeling inundated with information or temporary solutions.
